I’ve spent the last five years managing vigilance reports, PSURs, and EUDAMED submissions for Class IIa/IIb devices; the result is a practical appreciation for why the gap exists — and what manufacturers can reasonably do while the system remains fragmented.&lt;/p&gt;

  The gap in plain terms

&lt;p&gt;MAUDE is a central, public adverse-event repository. In the EU we do have strong legal frameworks for vigilance and post-market surveillance — see Chapter VII of MDR 2017/745 — but the data flows are distributed:&lt;/p&gt;

  Why there isn’t a MAUDE-equivalent (practical reasons)
&lt;/h2&gt;

&lt;p&gt;To be fair, the absence isn’t solely bureaucratic laziness. Several concrete factors combine:&lt;/p&gt;

&lt;li&gt;Fragmented legal responsibilities: Member States retain core vigilance tasks. Reporting goes to NCAs and then — depending on the event — to other Member States, the Commission, and economic operators. Different organisations, different systems.&lt;/li&gt;
&lt;li&gt;Phased IT implementation: EUDAMED was designed to centralise identifiers, certificates, vigilance, and market surveillance data. Its modules were delivered incrementally; adoption and public-facing functionality vary.&lt;/li&gt;
&lt;li&gt;Confidentiality and commercial sensitivity: Manufacturers and notified bodies legitimately argue that releasing granular reports can reveal proprietary designs, supplier details, or confidential corrective actions. Those concerns influence what NCAs publish.&lt;/li&gt;
&lt;li&gt;Heterogeneous publication policies: Some NCAs publish FSCA notices and summaries; others publish less. Language differences and redaction practices further limit utility.&lt;/li&gt;
&lt;li&gt;Resource constraints: Smaller NCAs or national IT projects may lack the budget or staff to operate public dashboards and to normalise multilingual reports into a single, searchable format.&lt;/li&gt;
&lt;/ul&gt;

  What this looks like day-to-day
&lt;/h2&gt;

&lt;p&gt;When an investigator calls asking whether our device has had X-type failures, the workflow is rarely simple:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Check internal vigilance log and PSUR summaries (we have them).&lt;/li&gt;
&lt;li&gt;Pull FSCA notices on our website and any communications to distributors/clinicians.&lt;/li&gt;
&lt;li&gt;Search NCA websites manually for public FSN (field safety notice) uploads in a handful of languages.&lt;/li&gt;
&lt;li&gt;Rely on notified-body feedback only if the issue touched conformity assessment — which is often not the case.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;This manual triage wastes time and creates inconsistency in what external stakeholders hear.&lt;/p&gt;

  Short-term manufacturer tactics that actually survive audits
&lt;/h2&gt;

&lt;p&gt;Until a single, public EU database exists in practice, manufacturers can reduce opacity for users and strengthen regulatory posture. Practical steps I use:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Publish FSN/FSCA summaries on your website, in English and the major national languages of your markets.&lt;/li&gt;
&lt;li&gt;Maintain a public-facing vigilance summary for each device family: a short timeline of significant incidents and actions (redacted where needed).&lt;/li&gt;
&lt;li&gt;Link clinical PMCF summaries or synopses to product pages — PSURs themselves are not public, but an executive summary is useful and audit-friendly.&lt;/li&gt;
&lt;li&gt;Use your eQMS to create connected workflows: link vigilance reports to CAPA, change control, risk assessment, and customer communications so you can produce consolidated narratives quickly for NCAs and clinicians.&lt;/li&gt;
&lt;li&gt;When possible, coordinate with distributors and hospitals to push FSNs to users rather than relying on NCA publication alone.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;These are low-tech, high-value measures: they improve transparency without exposing proprietary process details.&lt;/p&gt;

  Why transparency matters beyond optics
&lt;/h2&gt;

&lt;p&gt;Lack of central visibility isn’t just inconvenient. It affects patient safety and market surveillance:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Duplicate reports across Member States can be missed as signals if nobody aggregates them.&lt;/li&gt;
&lt;li&gt;Clinicians may reassign blame to devices prematurely without seeing manufacturer mitigations.&lt;/li&gt;
&lt;li&gt;Recalls or mitigations can be delayed simply because stakeholders are not aware of the same evidence.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;A clearer public picture reduces unnecessary alarm and helps regulators and manufacturers prioritise real risks.&lt;/p&gt;

  Where sensible compromise could live
&lt;/h2&gt;

&lt;p&gt;A workable European model need not mirror MAUDE exactly. Practical design choices:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Public executive summaries only (structured, anonymised) rather than full incident narratives containing supplier names or internal corrective plans.&lt;/li&gt;
&lt;li&gt;Strong UDI/Device Identification in EUDAMED combined with standardised taxonomy for incidents, to allow signal detection without revealing commercial details.&lt;/li&gt;
&lt;li&gt;A staged, searchable FSN register for devices of highest risk (implantables, class III) while lower-risk devices remain aggregated.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;These balance transparency and commercial confidentiality — and they align with MDR’s push for public-facing summaries for higher-risk devices.&lt;/p&gt;

  Final practical note for small manufacturers
&lt;/h2&gt;

&lt;p&gt;If your notified-body audit is in three months, focus on traceability: can you pull a thread from a single incident to the final FSN, CAPA, and PMCF action? An eQMS with connected workflow (traceability, automated CAPAs, AI-assisted impact analysis where you trust it) will save you hours in evidence-gathering and produce consistent public-facing summaries faster.&lt;/p&gt;

&lt;p&gt;To be fair, the EU’s approach is cautious for good reasons. But in five years of dealing with vigilance across multiple Member States, I’ve seen how opacity adds friction to both safety and compliance.&lt;/p&gt;

  What “AI” typically means in a QMS — operational, not magical
&lt;/h2&gt;

&lt;p&gt;Most eQMS vendors use AI in a narrow, operational way. In practice this means:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Natural-language search across documents (one search across the entire QMS), with semantic matching so you find the right procedure, risk assessment or CAPA even if wording differs.&lt;/li&gt;
&lt;li&gt;Assisted impact analysis: the system suggests which documents, products or processes a change might affect — often surfaced as a linked list you can review.&lt;/li&gt;
&lt;li&gt;Drafting assistance: auto-populating sections of a CAPA, nonconformance report or change request based on prior similar records.&lt;/li&gt;
&lt;li&gt;Triage and prioritisation: scoring incoming complaints or NCRs by keywords and historical outcomes to suggest priority.&lt;/li&gt;
&lt;li&gt;Pattern detection in structured fields: flagging rising trends in supplier nonconformances or repeated inspection findings.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;Call this “operational AI”. It speeds routine work and makes connected workflow usable. It is not doing your root-cause analysis for you. It is controlled assistance, not replacement of judgement.&lt;/p&gt;

  Common marketing claims — how to read them
&lt;/h2&gt;

&lt;p&gt;Vendors love short phrases. Read them carefully.&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;“Automates CAPA” — realistic translation: reduces manual data entry and suggests actions; you still need a human to own root cause, accept corrective action and verify effectiveness (ISO 13485:2016 requires documented evidence of effectiveness).&lt;/li&gt;
&lt;li&gt;“Self-healing QMS” — unrealistic. QMSs do not repair processes; they support human actors to do it faster.&lt;/li&gt;
&lt;li&gt;“Fully automated regulatory submissions” — partially true for pre-populated fields and export helpers; full regulatory narrative, clinical evidence and sign-off remain human responsibilities (per MDR requirements for clinical evaluation and technical documentation; Annex II is explicit about content and justification).&lt;/li&gt;
&lt;li&gt;“AI reviews your Technical File” — useful for spotting obvious gaps, but an AI cannot replace an expert review against Annex II/Annex IX expectations or notified body interpretation.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;When a vendor uses the word AI, ask: which of the above operational capabilities are implemented, and how are outputs logged and reviewed?&lt;/p&gt;

&lt;h2&gt;
  
  
  What regulators and notified bodies will expect
&lt;/h2&gt;

&lt;p&gt;In audits and conformity assessments I see three recurring themes:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Traceability and reviewability: every AI-generated suggestion must be traceable to source records and show who reviewed or overruled it. Audit trails are essential — store prompts, suggested text, and final approved text.&lt;/li&gt;
&lt;li&gt;Validation and acceptance criteria: treat AI features as software tools. Define acceptance tests and performance thresholds in your software verification plan (ISO 13485 clause 4.1 and design control principles apply in spirit).&lt;/li&gt;
&lt;li&gt;Risk analysis: include AI-driven behaviours in your risk management (ISO 14971). If an AI suggestion feeds a CAPA that changes production controls, the chain of influence must be assessed.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;In practice this means documenting the AI feature in your Change Control, updating your Risk Management File, and demonstrating to your notified body that users are trained and outputs are reviewed.&lt;/p&gt;

  Practical controls I insist on
&lt;/h2&gt;

&lt;p&gt;I push for the following minimum controls whenever we deploy AI features in the QMS:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Prompt and output logging: save the user prompt, the AI response, and who accepted/edited it.&lt;/li&gt;
&lt;li&gt;Human-in-the-loop sign-off: no AI-generated text is final until a named person signs it off.&lt;/li&gt;
&lt;li&gt;Reproducibility tests: run the same prompt periodically and on software updates to ensure behaviour is consistent or changes are documented.&lt;/li&gt;
&lt;li&gt;Acceptance criteria: measurable tests for suggested mappings (e.g., precision/recall thresholds for document linkage) that you validate during rollout.&lt;/li&gt;
&lt;li&gt;Versioned models and update policy: vendors should state when models are updated and what validation is performed — this goes into Change Control.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;These controls make the AI feature auditable and link it to your overall QMS, which is what inspectors actually look for.&lt;/p&gt;

&lt;h2&gt;
  
  
  Where AI gives the most tangible ROI
&lt;/h2&gt;

&lt;p&gt;If you need to prioritise, these are the areas where I’ve seen real, low-risk benefit:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Finding the right evidence quickly — faster literature searches and semantic search across Technical Files.&lt;/li&gt;
&lt;li&gt;Faster impact mapping for changes — a suggested map saves hours of manual tracing.&lt;/li&gt;
&lt;li&gt;Reducing form friction — auto-filled fields cut the time to open a CAPA and improve data consistency.&lt;/li&gt;
&lt;li&gt;Trend detection for surveillance — catching repeating supplier issues sooner so you can open fewer large CAPAs later.&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;These are valuable because they integrate into existing workflows and preserve reviewer responsibility.&lt;/p&gt;

&lt;h2&gt;
  
  
  Features I remain sceptical about
&lt;/h2&gt;

&lt;ul&gt;
&lt;li&gt;AI that claims to “decide” severity or regulatory classification without human review. Classification decisions (e.g., under MDR) have legal implications and need a named responsible person.&lt;/li&gt;
&lt;li&gt;Black-box recommendations with no explainability. If you cannot trace why a document was linked or why a priority was set, you will struggle in an audit.&lt;/li&gt;
&lt;li&gt;Claims of full automation for clinical evaluation or PMCF design. AI can assist literature screening or draft study outlines, but you must retain clinical science oversight.&lt;/li&gt;
&lt;/ul&gt;

  Final practical advice
&lt;/h2&gt;

&lt;p&gt;Treat AI features as you would any other tool that affects product safety or documentation. Document the feature in your QMS, run defined validation, preserve audit trails, and mandate human sign-off. The phrase I use with vendors now is “operational AI, controlled assistance, traceable outcome” — that’s what gets through an audit.&lt;/p&gt;

&lt;li&gt;Cybersecurity and real-world performance. Regulators expect post-market monitoring of vulnerabilities and real-world performance metrics, but many companies have a developer-centric patch workflow, not a regulated post-market plan.&lt;/li&gt;
&lt;li&gt;Traceability and impact analysis. Auditors want to see links: requirement → hazard analysis → verification → clinical data → post-market actions. Too often these links are implicit, scattered across tools, or missing entirely.&lt;/li&gt;
&lt;/ul&gt;

  Why this matters (beyond paperwork)
&lt;/h2&gt;

&lt;p&gt;Treating the gap as mere bureaucracy misses the point. SaMD updates change clinical behaviour: how clinicians interpret an output, how a workflow runs, how an alarm looks. If you can’t show you considered the risk and validated performance, a notified body will either slow you down or require post-market studies you’re not prepared for. I’ve watched teams face months of delay because a routine UI tweak was classified as a change requiring additional clinical evidence.&lt;/p&gt;

  Practical adjustments that actually work
&lt;/h2&gt;

&lt;p&gt;These are the things I insist on early, before a design review or a CE submission:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;Map intended purpose at the function level. Don’t stop at “diagnostic support”; list each algorithmic output, who uses it, and the clinical decision it influences. This is the single clearest way to resolve Rule 11 ambiguity.&lt;/li&gt;
&lt;li&gt;Perform software-specific risk analysis (ISO 14971 + IEC 62304). Include use-related hazards and consider failure modes for updated algorithms. In practice this means a software hazard table tied to requirements.&lt;/li&gt;
&lt;li&gt;Predetermine change-control plans. Define categories of change (e.g., security patch vs algorithm weight update) and the required evidence per category: unit tests, integration tests, clinical re-validation, PMCF entry. This mirrors the “predetermined change control” approach auditors like to see.&lt;/li&gt;
&lt;li&gt;Build traceability early. Link requirements → design → verification/validation → clinical evidence → release notes. If you use an eQMS, native workflow integration that shows these links saves hours in an audit.&lt;/li&gt;
&lt;li&gt;Design PMCF and performance monitoring into release. For SaMD, plan telemetry, usage metrics, false-positive/negative logging, and a dashboard that feeds your PSUR/PMCF analysis.&lt;/li&gt;
&lt;li&gt;Talk to your notified body early. Share your function map and change categories. You’ll get different answers; capture them and treat them as part of your risk acceptance/justification.&lt;/li&gt;
&lt;/ul&gt;

  A small checklist for your next sprint
&lt;/h2&gt;

&lt;ul&gt;
&lt;li&gt;Have you defined the intended purpose at function level?&lt;/li&gt;
&lt;li&gt;Is each function mapped to a classification rationale under Rule 11?&lt;/li&gt;
&lt;li&gt;Do you have software hazard analysis and traceability to verification?&lt;/li&gt;
&lt;li&gt;Is there a predetermined change-control plan for software updates?&lt;/li&gt;
&lt;li&gt;Are telemetry and clinical performance metrics specified and collected?&lt;/li&gt;
&lt;li&gt;Can you demonstrate how a patch or algorithm change would flow through your QMS (change → risk assessment → validation → release)?&lt;/li&gt;
&lt;/ul&gt;

&lt;p&gt;If you use an eQMS, look for features that make these concrete: automatic traceability, change-impact mapping, connected workflow for CAPAs and changes, and built-in artefacts for PMCF/PSUR. Automated CAPAs and AI-guided assistance are useful — but only if the outputs are reviewable and traceable. Controlled assistance, not magic, is what passes audits.&lt;/p&gt;

  Final note — on notified bodies and reality
&lt;/h2&gt;

&lt;p&gt;Notified bodies want to protect patients; the variability comes from translating new software realities into a regulatory framework. To be fair, the guidance is catching up (IMDRF principles, MDCG documents on software classification), but the practical work remains on manufacturers: be explicit, be auditable, and treat updates as regulated events. Like choosing the right route before you set off on a steep alpine climb, choosing the right documentation strategy before your next major software release saves a lot of backtracking.&lt;/p&gt;
